Conditions you may be admitted for:

Cancer
Any patient whose cancer is metastatic or inoperable Cancer
  • Uncontrolled signs and symptoms due to cancer or treatment
  • Terminal disease at diagnosis
  • Disease progesses despite therapy
Heart Disease
  • Stage III or IV heart failure despite optimal medical management
  • Angina refractory to medical or interventional management
  • Heart failure symptoms at rest/Ejection fraction<20%
  • New dysrhythmia / Cardiac arrest or syncope
  • Frequent emergency visits or hospital admissions
Dementia
  • Behavioral problems
  • Feeding problems / weight loss
  • Caregiver stress
  • Increased safety concerns
  • Unable to walk, bathe or dress
    self without assistance
  • Incontinence
  • Less than six intelligible words
  • Frequent emergency visits
Pulmonary Disease
  • Oxygen-dependent, o2 sats <88% on room air
  • PCO2>50
  • Dyspnea with minimal to moderate
    exertion / at rest
  • Others, e.g pulmonary fibrosis,
    pulmonary hypertenstion
  • Signs or symtoms of right heart failure
  • Unintentional weight loss
Neurologic
  • Frequent emergency room visits
  • Stroke
  • Parkinson's
  • Albumin<2.5
  • Unintentional weight loss
  • Decubitus ulcer
  • Homebound / bed confined
Liver Disease
  • Refractory ascities / SBP
  • Symptomatic disease /
    jaundice / confusion
  • Increased safety concerns
  • Labs: INR>5 Albumin<2.5
  • Malnutrition and muscle wasting
    Malnutition and muscle wasting
Debility / Failure to Thrive
  • More three chronic conditions
    in oatient over 75 year old
  • Clinical progreesion of
    disease / Functional decline
  • Weight loss
  • Patient/Family desire for
    low-yeild therapy
  • No desire / a candidate for
    aggressive treatment
  • Frequent emergency
    visits / hospitalizations
Renal Disease
  • Dialysis / not a candidate for dialysis
  • Stage IV or Stage V chronic kidney Disease
  • Creatinine Clearance <15mL/minute
  • Serum creatinine >6.0
